COVID-19 Vaccine Reporting Form

The purpose of this form is to gather vaccine information from health care providers for vaccine inventory management and mandatory ministry reporting. Every administered, extra, and wasted dose of COVID-19 vaccine MUST be documented in COVax and captured in this form.

**PLEASE NOTE**: if you used more than one Lot Number per vaccine product, you must complete an additional survey for each Lot Number.
